VHL, HIF-2alpha (EPAS1), and KEAP1-NRF2 Hypoxia and Oxidative Stress Sensing
VHL (CRL2-VHL) and KEAP1 (CRL3-KEAP1) are cullin-RING E3 ubiquitin ligase substrate receptors that continuously degrade HIF-1alpha/HIF-2alpha (EPAS1) under normoxia and NRF2 (NFE2L2) under basal redox states, coupling oxygen and electrophile sensing to transcription.
Metazoan cells maintain continuous surveillance over intracellular molecular oxygen (O2) tension and reactive electrophile/oxidative balance through two rapid post-translational cullin-RING E3 ubiquitin ligase (CRL) degron switches: the VHL–HIF-1α/HIF-2α (EPAS1) oxygen-sensing axis and the KEAP1–NRF2 (NFE2L2) oxidative stress-sensing axis. Rather than waiting hours to synthesize transcription factors de novo when hypoxia or oxidative stress strikes, vertebrate cells constitutively translate the master basic helix-loop-helix–PAS transcription factors HIF-1α (HIF1A) and HIF-2α (EPAS1) alongside the basic leucine-zipper (bZIP) factor NRF2 (NFE2L2), while simultaneously targeting them for rapid 26S proteasomal destruction (t1/2 < 5–15 minutes) under basal physiological conditions via the E3 ligase substrate adaptors pVHL (Von Hippel–Lindau tumor suppressor, encoded on human chromosome 3p25.3) and KEAP1 (Kelch-like ECH-associated protein 1, encoded on chromosome 19p13.2).
When cellular O2 tension drops below physiological thresholds or electrophilic reactive oxygen species (ROS) modify sensor cysteines, enzymatic hydroxylation by prolyl hydroxylase domain (PHD1–3 / EGLN1–3) dioxygenases or conformational gating of the KEAP1 Kelch β-propeller ceases. Nascent HIF-2α and NRF2 escape ubiquitylation, accumulate in the nucleus within minutes, and heterodimerize with ARNT (HIF-1β) or small MAF proteins to activate angiogenic, erythropoietic, glycolytic, and glutathione-antioxidant transcription programs. Somatic mutations that break these two switches—biallelic VHL inactivation in >85% of clear-cell renal cell carcinomas (ccRCC) and KEAP1/NFE2L2 mutations in ~20–25% of non-small-cell lung carcinomas (NSCLC)—constitute premier drivers of metabolic reprogramming, angiogenesis, and therapy resistance in human oncology.
The VHL–PHD–HIF-2alpha (EPAS1) Oxygen-Sensing Pathway
Prolyl Hydroxylase (EGLN1–3) Dioxygenase Chemistry and CRL2-VHL Degron Recognition
Under normoxic conditions (pO2 ≈ 20–100 mmHg), the oxygen-dependent degradation domains (ODDD) of HIF-1α (Chr14q23.2, 826 aa) and HIF-2α (EPAS1, Chr2p21, 870 aa) are hydroxylated on two conserved proline residues (Pro402 and Pro564 in HIF-1α; Pro405 and Pro531 in HIF-2α) within an LxxLAP motif by the non-heme Fe2+- and 2-oxoglutarate (α-ketoglutarate)-dependent dioxygenases PHD1 (EGLN2), PHD2 (EGLN1), and PHD3 (EGLN3). In this reaction, one atom of dioxygen (O2) is incorporated directly into the C4 trans-hydroxyl group of proline (4-hydroxyproline, Hyp), while the second oxygen atom oxidatively decarboxylates 2-oxoglutarate into succinate and CO2, with ascorbate (vitamin C) maintaining the catalytic active-site iron in the reduced Fe2+ state.
Because PHD2 (EGLN1) exhibits a high apparent Michaelis constant for O2 (Km ≈ 230–250 μM, above dissolved atmospheric O2 concentrations in tissues), its reaction velocity scales almost linearly with physiological O2 partial pressure. Trans-4-hydroxylation of Pro564/Pro531 introduces two critical hydrogen bonds with Tyr98 and His115 inside the β-domain pocket of pVHL (213 amino acids, forming the CRL2^VHL complex with Elongin B, Elongin C, Cullin-2, and RBX1), increasing pVHL binding affinity by >1,000-fold (Kd ≈ 30 nM). A second Fe2+/2-oxoglutarate dioxygenase, Factor Inhibiting HIF (FIH1 / HIF1AN), hydroxylates Asn803 (HIF-1α) or Asn847 (HIF-2α) in the C-terminal transactivation domain (CAD) to block recruitment of the p300/CBP histone acetyltransferase coactivator.
Clear-Cell Renal Cell Carcinoma (ccRCC), Belzutifan, and Tibetan EPAS1 Adaptation
When hypoxia (O2 < 1–2%) or biallelic loss of VHL on chromosome 3p25.3 (via Chr3p chromothripsis/deletion plus somatic point mutation or promoter DNA methylation) eliminates CRL2^VHL recognition, unhydroxylated HIF-2α accumulates and heterodimerizes with ARNT (HIF-1β) via tandem basic helix-loop-helix (bHLH) and Per-ARNT-Sim (PAS-A and PAS-B) domains to bind Hypoxia Response Elements (HRE, 5′-RCGTG-3′). In clear-cell renal cell carcinoma (ccRCC), HIF-2α acts as the dominant oncogenic driver (whereas HIF-1α is frequently deleted on Chr14q), activating VEGFA (vascular endothelial growth factor A), PDGFB, TGF-α, CCND1 (cyclin D1), and SLC2A1 (GLUT1).
Although transcription factors are historically difficult to drug, crystallographic studies revealed an unusually large, preformed ~290 ų internal hydrophobic cavity inside the PAS-B domain of HIF-2α (absent in HIF-1α). Structure-based drug discovery yielded the oral allosteric inhibitor belzutifan (PT2977 / MK-6482), which binds inside the HIF-2α PAS-B cavity (Ki ≈ 2–5 nM) and induces an allosteric conformational shift in β-sheet interface residues (Met252, His293) that disrupts HIF-2α–ARNT heterodimerization, earning FDA approval for VHL disease-associated tumors and advanced ccRCC. Conversely, in human evolutionary genomics, high-altitude Tibetan populations carry an introgressed Denisovan haplotype across the EPAS1 (HIF-2α) locus alongside gain-of-function EGLN1 (PHD2 Asp4Glu/Cys127Ser) variants that dampen hypoxic EPO (erythropoietin) overproduction at >4,000 meters elevation, preventing chronic mountain sickness, polycythemia, and pulmonary hypertension under natural selection.

The KEAP1–NRF2 (NFE2L2) Oxidative and Electrophile Sensing Switch
Hinge-and-Latch (ETGE/DLG) Mechanics and Reactive Cysteine Sensors
In parallel with oxygen sensing, the KEAP1–NRF2 axis governs cellular defense against xenobiotic electrophiles, lipid peroxides (ferroptosis), and oxidative stress during oxidative phosphorylation. KEAP1 (624 amino acids) is a homodimeric substrate adaptor for the Cullin-3 (CRL3^KEAP1) E3 ubiquitin ligase consisting of an N-terminal BTB (Broad-Complex, Tramtrack, and Bric-a-brac) homodimerization domain that binds CUL3, a cysteine-rich Intervening Region (IVR / BACK domain), and a C-terminal 6-bladed Kelch β-propeller (DGR) domain.
In the unstressed basal state, one KEAP1 homodimer bridges a single molecule of NRF2 (NFE2L2, 605 amino acids) via its N-terminal Neh2 degron domain using a two-site "hinge-and-latch" mechanism: one Kelch β-propeller binds the high-affinity ETGE motif (residues 79–82, Kd ≈ 5 nM, the hinge), while the second Kelch β-propeller captures the lower-affinity DLG motif (residues 29–31, Kd ≈ 1 μM, the latch). This dual-site clamping positions a central α-helix containing seven lysine residues of NRF2 directly before the RBX1-bound E2 ubiquitin-conjugating enzyme, driving rapid polyubiquitylation and 26S proteasomal degradation (t1/2 ≈ 15 minutes).
KEAP1 senses oxidative and electrophilic stress through a constellation of hyper-reactive surface cysteine thiolates with depressed pKa values, principally Cys151 (located in the BTB–CUL3 interface) and Cys273 / Cys288 (located in the IVR domain), plus Cys226/Cys613/Cys622/Cys624 (which sense H2O2 and heavy metals via disulfide formation). Covalent Michael addition or sulfenylation of Cys151 or Cys273/Cys288 by endogenous metabolites (fumarate, itaconate) or electrophilic drugs (dimethyl fumarate used in multiple sclerosis, omaveloxolone approved for Friedreich ataxia, or sulforaphane) induces an allosteric conformational shift in the KEAP1 homodimer that uncouples ubiquitylation without releasing NRF2 from the ETGE hinge. Newly synthesized NRF2 bypasses the saturated KEAP1 pool, translocates to the nucleus, and heterodimerizes with small MAF proteins (MAFF, MAFG, MAFK) at Antioxidant Response Elements (ARE, 5′-TGACnnnGC-3′).
Metabolic Reprogramming and Glutaminase Sensitivity in KEAP1-Mutant NSCLC
In ~20–25% of lung adenocarcinomas (LUAD) and lung squamous cell carcinomas (LUSC), somatic constitutive activation of NRF2 occurs via either biallelic loss-of-function mutations scattered across the KEAP1 Kelch and BTB domains or recurrent gain-of-function hotspot missense mutations within the NFE2L2 DLG (Trp24, Asp27, Asp29, Leu30, Gly31) and ETGE (Glu79, Thr80, Gly81, Glu82) degron motifs. In LUAD, KEAP1 mutations frequently co-occur with STK11 (LKB1) and KRAS mutations, defining an aggressive, radiation-resistant, and anti-PD-1 immunotherapy-refractory clinical subtype.
Constitutive NRF2 transcriptionally upregulates over 200 cytoprotective and metabolic genes:
- Glutathione Biosynthesis and Cystine Import: Upregulation of the cystine/glutamate antiporter
SLC7A11(xCT), glutamate-cysteine ligase catalytic/modifier subunits (GCLC/GCLM), and glutathione reductase (GSR), shielding tumor cells from radiation-inducedROSand lipid-peroxidation ferroptosis. - NADPH Generation via the Pentose Phosphate Pathway (
PPP): Activation ofG6PD,PGD,ME1(malic enzyme 1), andIDH1to supply cytosolicNADPHfor thioredoxin (TXN/TXNRD1) and glutathione regeneration. - Collateral Glutamate Depletion and
GLS1/NR0B1Vulnerabilities: Because highSLC7A11expression exports large fluxes of intracellular glutamate in exchange for extracellular cystine (at a 1:1 stoichiometric ratio) whileGCLCconsumes glutamate to synthesize glutathione (GSH),KEAP1-mutant tumor cells suffer chronic intracellular glutamate depletion—rendering them selectively dependent on exogenous glutamine catabolism via glutaminase (GLS1) or on theNRF2transcriptional cofactorNR0B1(DAX1).
Frequently Asked Questions
Why do somatic NFE2L2 (NRF2) mutations cluster exclusively in Exon 2 (DLG and ETGE motifs) whereas KEAP1 mutations are distributed across the entire coding sequence?
Because NFE2L2 functions as a dominant oncogene, gain-of-function mutations must disrupt KEAP1 binding (DLG latch or ETGE hinge in Exon 2) while leaving the C-terminal Neh1 DNA-binding (bZIP) and Neh4/Neh5 transactivation domains intact. Conversely, KEAP1 acts as a tumor suppressor whose 624-residue polypeptide must maintain dimeric BTB geometry, IVR linker stiffness, and a six-bladed Kelch β-propeller; loss-of-function missense or truncating mutations anywhere along the KEAP1 gene can disrupt CRL3^KEAP1 ubiquitylation.
How do clinical PHD inhibitors (such as Roxadustat and Daprodustat) treat renal anemia without causing VHL tumors?
Oral prolyl hydroxylase inhibitors (HIF-PHIs such as roxadustat, daprodustat, and vadadustat) are reversible, short-acting 2-oxoglutarate mimetics that coordinate the catalytic Fe2+ ion of PHD1–3. Intermittent dosing transiently stabilizes HIF-2α in renal peritubular interstitial fibroblasts and hepatocytes for a few hours to stimulate endogenous erythropoietin (EPO) transcription and iron absorption (by suppressing hepcidin), avoiding the continuous, multi-year, high-amplitude HIF-2α activation driven by biallelic VHL deletion in ccRCC.
How does fumarate hydratase (FH) or succinate dehydrogenase (SDH) deficiency mimic VHL loss ("pseudohypoxia")?
In hereditary leiomyomatosis and renal cell cancer (HLRCC, caused by fumarate hydratase FH loss) and paraganglioma/pheochromocytoma syndromes (caused by succinate dehydrogenase SDHA–D loss), mitochondrial tricarboxylic acid (TCA) cycle blockades cause millimolar accumulation of intracellular fumarate or succinate. Both metabolites act as competitive product inhibitors of 2-oxoglutarate-dependent dioxygenases (PHD1–3 as well as TET DNA demethylases and KDM histone demethylases), stabilizing HIF-1α/HIF-2α under normoxia; moreover, accumulated fumarate directly succinates KEAP1 cysteines (2-succinyl-cysteine), simultaneously activating NRF2.
